Clinical information managers (CIMs)® are medical scribes with a unique opportunity to work closely with attending physicians and the entire healthcare team in emergency departments (EDs). The position is popular among those who aspire to become physicians, Physician assistants, nurses, nurse practitioners, EMTs or healthcare administrators.
 
CIMs work closely with our ED physicians to document physician notes for each patient in a state-of-the-art electronic medical record system. To obtain the history and physical exam findings, CIMs may accompany physicians into the patient rooms and take notes. Physicians also may dictate information to CIMs, who are based at computer terminals in dictation rooms or at the nurses’ stations.    
 
CIMs transcribe vital signs, medications and allergies as well as patient orders (such as medications, IVs). They also document procedures performed in the ED, consultations or advice obtained by physicians, and conversations with other physicians or patients' family members.  Under the direction of physicians, CIMs may document discharge instructions, prescriptions or diagnoses in charts.
 
CIMs keep track of diagnostic tests ordered and will inform physicians when results are returned to the department; this hastens physicians' decision-making processes. CIMs also obtain physicians' interpretation/reading of diagnostic studies (such as EKGs and x-rays) and enter that information into physician notes.
